生殖過程是一個復雜而精細的生理流程,包括精子與卵子的形成、成熟、排卵、受精、著床以及胚胎發(fā)育等多個環(huán)節(jié)。通過阻斷其中任何一個環(huán)節(jié),都可以達到避孕或終止妊娠的目的。
男性節(jié)育藥物目前,在我國研究較為成熟的僅有一種男性節(jié)育藥物——棉酚。它是由棉花根、莖和種子中提取的一種黃色酚類物質(zhì)。使用后會減少精子數(shù)量;隨著用藥時間的延長,精子數(shù)量可以降至零。經(jīng)4000多例健康男子試用,其有效率高達99%以上。
女性避孕藥物根據(jù)作用環(huán)節(jié)的不同,女性避孕藥主要分為以下幾類:
抑制排卵的避孕藥:這是最常見的一類女性避孕藥物,由不同類型的雌激素和孕激素按不同劑量組成。主要包括短效口服避孕藥(如復方炔諾酮片、復方甲地孕酮片)和長效注射避孕藥(如復方己酸孕酮)。這類藥物可能會出現(xiàn)早孕反應、子宮不規(guī)則出血、閉經(jīng)、乳汁減少及凝血功能亢進等不良反應。對急慢性肝病患者、糖尿病患者及有子宮肌瘤的婦女不宜使用。
抗著床避孕藥:此類藥物通過干擾子宮內(nèi)膜正常發(fā)育和轉(zhuǎn)化,使其不利于孕卵著床。我國常用的大劑量炔諾酮或甲地孕酮即屬于此類藥物。這類藥物的一大優(yōu)點是應用不受月經(jīng)周期限制。
主要影響子宮和胎盤功能的避孕藥:目前正處于研究階段,尚未大規(guī)模使用。主要藥物為前列腺素類物質(zhì)。
